Admission Nurse
Key Responsibilities:
- Conduct initial assessments and admissions for hospice and home health patients.
- Identify patient needs, explain services, and obtain medical orders.
- Serve as a key communicator with referral sources, patients, families, physicians, and the interdisciplinary team.
- Document findings promptly and accurately.
- Deliver direct nursing care, including teaching, advocacy, pain and symptom management, and initiating interventions.
- Remain flexible for patient coverage and respond to emergencies.
- Participate in quality improvement, agency committees, and community outreach.
- Maintain confidentiality and professional boundaries.
RN Case Manager
Key Responsibilities:
- Manage and coordinate nursing care for assigned terminally ill and palliative care patients.
- Assess needs, implement and revise nursing care plans, and evaluate outcomes.
- Deliver direct patient care, including pain/symptom management, teaching, advocacy, and monitoring.
- Supervise and instruct LPNs, Home Health Aides, and CNAs.
- Participate in patient care conferences and interdisciplinary team meetings.
- Document findings and care in a timely manner.
- Anticipate and address patient and family needs, including emergencies and bereavement follow-up.
- Participate in quality improvement, agency committees, and student nurse programs.
- Maintain confidentiality and professional boundaries.
Qualifications
- Graduate of an accredited school of nursing.
- Current RN license to practice in Indiana.
- CPR certification (or willingness to obtain during orientation).
- Infusion skills and recent hospital or home care experience preferred.
- Ability to function independently and travel between patient homes.
- Strong assessment, case management, and interpersonal skills.
- Comfort working with hospice and palliative care patients.
