Physical Therapist (PT) – Home Health | Olean, NY | PRN
Stern at Home Therapy is hiring a PRN Physical Therapist (PT) to provide one-on-one care to patients throughout Olean, NY, and surrounding communities.
This opportunity is ideal for a Physical Therapist looking to supplement an existing schedule, maintain a small caseload, or add flexible home health work while maintaining control over when and where they see patients.
With a minimum commitment of just 5 patients, our PRN option provides flexibility while allowing therapists to build a geographically concentrated caseload within the Olean area.
What You'll Do:
As a Physical Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population in their homes.
You'll maintain clinical autonomy over evaluations and treatment plans while our administrative team assists with referrals, scheduling, authorizations, and patient coordination.
Responsibilities include:
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one physical therapy treatments in patients' homes.
- Help patients improve strength, balance, mobility, endurance, and functional independence.
- Provide gait training, transfer training, therapeutic exercise, and fall-prevention strategies.
- Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
- Educate patients and caregivers on exercises, mobility strategies, and safety.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with physicians, caregivers, families, and interdisciplinary care team members.
Compensation:
- PRN 1099 – Minimum 5 Patients.
- $62 per follow-up visit.
- $75 per evaluation.
- Fixed per-visit compensation with no tiered volume model.
- PRN positions are 1099 independent contractor roles only.

Qualifications:
- Active Physical Therapist license in New York required.
- Graduate of an accredited Physical Therapy program.
- Reliable transportation required.

- Home health or geriatric experience preferred but not required.
