Job Description
The PACE Home Care Coordinator (HHA)-Permanent/ Full Time is a key member of the Interdisciplinary Team (IDT), working under the supervision of the PACE Home Care Coordination RN Manager. This role ensures seamless coordination of home-based services for our elderly participants including caregiver scheduling, delivery of medical equipment, and meal coordination.
Title: PACE Home Care Coordinator-(HHA) - Permanent/Full Time
Location: San Ysidro, CA 92173
Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Compensation: $22.39 – $28.09 per hour Benefits
Key Responsibilities
- Conduct home safety evaluations (pre-enrollment, annual, and address changes).
- Coordinate delivery of meals, incontinence supplies, DME, and caregiver services.
- Track and document services in EMR.
- Collaborate with the IDT to align services with participant goals.
- Conduct monthly visits for homebound and RCFE participants.
- Support PACE nurses with logistics and participant care needs.
- Generate reports and assist with participant satisfaction surveys.
Minimum Qualifications
- Home Health Aide (HHA) Certificate from CA Dept. of Public Health
- Certified Nursing Assistant (CNA)
- CPR & First Aid Certification (American Heart Association)
- Valid California Driver’s License
- One (1) year of experience working with frail or elderly population
- Basic computer skills (Microsoft Outlook, Excel)
