Job Summary
Seeking experienced Registered Nurses (or LVNs) to join the Transfer Center–Clinical team. This position involves zero hands-on patient care. Clinicians will facilitate patient transfers, coordinate with hospitals and providers, and ensure safe and efficient movement of patients within the Dignity system and external facilities.

Location: Rancho Cordova, CA 95670
Duration: 13-Week Contract
Shift: Varied (1445–0315 and 1245–0115), Every Other Weekend
Unit Information
- Unit: Transfer Center – Clinical (RN/LVN)
- Setting: Acute / Emergency (ER)
- Charting System: Cerner
- Bed Management: TeleTracking
- Avg Case Load: ~12 cases per shift (10–15 active at once)
Key Responsibilities
- Facilitate transfers for all patient types within and outside of Dignity Health.
- Complete patient assessments including vitals and clinical information after intake.
- Locate accepting physicians and specialists; request beds from placement teams.
- Communicate with hospitals across the system.
- Use multiple applications (Cerner, TeleTracking, Tiger Connect, Google Suite).
- LVNs: Primary responsibility is “refer-outs” when appropriate placement cannot be found within Dignity.
Required Experience & Skills
- Minimum 2 years of ICU, ED, or Transfer Center experience
- Must have acute care experience
- Strong critical care assessment and clinical judgement
- Ability to follow algorithms, multitask, and learn quickly
- Excellent communication and customer service skills
- Proficiency with multiple software applications
- Cerner or Epic required
- BLS required
- First-time travelers welcome
Licensure
- Active RN or LVN license
- License to submit: Yes
- Required Prior to Acceptance:
- DYS Exam through UNA
- Screenshot of attempts required
- Relias Dys exam within 12 months accepted
- Failure of the second DYS attempt = DNU for CommonSpirit AZ & Sacramento
Scheduling & Requirements
- Available any day of the week
- Every other weekend required
- Holiday work may be required
- No floating
- Training:
- 1 day hospital orientation
- 3–5 days unit orientation (any shift)
