Hospital Liaison/Marketer

Sterling Care in Harford County, MD, is currently seeking a Hospital Liaison to join our team.

Summary/Objective:

The Hospital Liaison serves as the primary representative for Sterling Care within assigned acute care hospitals. This individual is responsible for building and maintaining strong relationships with case managers, care navigators, social workers, physicians, and hospital leadership while identifying and converting appropriate skilled nursing referrals. The ideal candidate is highly motivated, relationship-driven, understands Medicare and managed care, and has previous Skilled Nursing Facility admissions experience.

Essential Functions

Works with interdisciplinary team members to identify short-term and long-range issues that must be addressed; recommends options and courses of action to address issues.

• Developes and strengthens hospital referral relationships, identifies appropriate skilled nursing candidates, conducts bedside consultations, reviews referrals, and partners with facility Admissions Directors to increase hospital admissions.
• Builds and maintains relationships with hospital case managers, social workers, physicians, and discharge planners.
• Conducts bedside visits with patients and families to educate them on Sterling Care’s scope of services.
• Reviews incoming referrals for clinical and financial appropriateness.
• Works closely with Admissions Directors to coordinate admissions and ensure timely placement.
• Understands Medicare, Managed Medicare, Medicaid, and insurance authorization requirements.
• Monitor hospital census and proactively identifies discharge opportunities.
• Maintain regular communication with referral sources and provides updates throughout the admission process.
• Travels daily between assigned hospitals, including Johns Hopkins Hospital, Johns Hopkins Bayview, and MedStar Franklin Square.

Preferred Qualifications:

• 3-5 years of Skilled Nursing Facility Admissions or Hospital Liaison experience.
• Strong understanding of Medicare, Managed Care, insurance authorizations, and SNF admissions.
• Experience reviewing referrals and determining clinical appropriateness.
• Proven ability to build and maintain hospital referral relationships.
• Excellent communication and follow-up skills.
• Highly organized, self-motivated, and able to work independently.
