Location: 706 N. Main St. Rockford IL, 61103
Schedule: Full-Time, 8am–5pm

We’re seeking a Rehabilitation Supervisor to lead, support, and inspire clinical teams across our programs.
What You'll Do:
- Oversee day-to-day operations of assigned mental health programs
- Supervise, support, and evaluate Program Managers, MHPs, and direct care staff
- Conduct and review clinical assessments and integrated treatment plans
- Ensure compliance with regulatory standards and agency policies
- Maintain documentation quality and lead regular team supervision and meetings
- Collaborate with multidisciplinary teams for client care coordination
Requirements
What You Bring:
- Education: Master's in Human Services OR Licensed RN
- Experience: 5 years in human services; 1 year in a supervisory/administrative role preferred
- License: Valid IL driver’s license, liability insurance, reliable vehicle
- Strong leadership, organization, and communication skills
- Qualified Mental Health Professional (per Medicaid Rule 140)
