This role combines in-home patient visits with remote care coordination, allowing much of your work, including documentation, communication, and care planning, to be completed from home.
Position Overview
The RN will provide skilled nursing services to clients in their homes while also supporting care coordination remotely. Visits are completed on a per-visit basis, with flexibility to accept assignments based on your availability.

Responsibilities
- Perform skilled nursing visits and assessments
- Provide patient education and medication management
- Complete timely and accurate documentation
- Coordinate care with physicians, families, and team members (remotely)
- Support individualized care planning
Compensation
- Routine Skilled Nursing Visit: $60 per visit
- Start of Care (SOC): $115–$125 per visit
- Recertification/Resumption Visits: $90–$100 per visit
- Discharge Visit: $75–$85 per visit
Additional time spent on care coordination, physician communication, or complex case management is compensated at $30/hour (as approved).
This role offers flexibility in scheduling and workload, allowing nurses to maintain work-life balance while earning competitive per-visit rates.
Qualifications
- Active RN license in Utah
- Strong communication and organizational skills
- Ability to work independently
- Home health experience preferred but not required
