Overview
We are seeking a compassionate and dedicated Home Health Aide to provide exceptional care for residents in a memory care assisted living setting. Hours for this role are fixed at 24-32 hours per week (depending on whether candidate is intertested in part time or full time status). The shift start and end time is 11:00 PM-7:00 AM with every other weekend and every other holiday.
This role involves supporting individuals with diverse health needs, promoting independence, and enhancing their quality of life. The ideal candidate will have a strong background in patient care, excellent communication skills, and a genuine desire to make a positive impact on clients' well-being. Prior experience in senior care, home health, or long-term care settings is highly valued. This position offers an opportunity to work closely with patients requiring memory care.
Duties
- Provide personal care including assisting with activities of daily living such as bathing, dressing, grooming, and medication administration
- Monitor and observe patient health status, including vital signs and symptoms related to dementia, Alzheimer's, or other chronic conditions
- Document patient information accurately using EMR/EHR systems while maintaining HIPAA compliance
- Recognize signs of medical emergencies or changes in condition and escalate appropriately
- Maintain a compassionate attitude when working with individuals with developmental disabilities or cognitive impairments such as dementia or memory loss
Experience
- Prior experience in home health, senior care, assisted living, or nursing homes preferred
- Familiarity with medical terminology and medical documentation practices
- Experience working with people with dementia or developmental challenges is advantageous
- Knowledge of EMR/EHR systems and HIPAA regulations is preferred
- Certification as CNA or TMA is preferred, but not requried. Opportunity for education assistance to complete these.
- Experience assisting with ADLs
- Demonstrated ability to work compassionately with elderly residents in assisted living setting
