Position Summary
The Outpatient Therapist, under the direction and supervision of the Clinical Director, is responsible for providing and coordinating therapeutic services for clients experiencing substance use and mental health concerns. The therapist will manage, support, and facilitate appropriate services to meet the individualized needs of clients and their families.
Duties and Responsibilities
- Complete Comprehensive Clinical Assessments (CCA)
- Assist individuals served in developing person-centered service plans that are current, goal-oriented, and reflective of consumer progress
- Provide case management and care coordination, including regular communication with clients, families, and providers
- Work collaboratively with professionals and community partners to support a team-based approach to care
- Keep the Clinical Team Leader and/or supervising professional staff informed of clinical and administrative issues related to service delivery
- Maintain timely and accurate documentation in accordance with agency and regulatory standards
Qualifications and Experience
- Master’s degree in a Human Services-related field required
- Full licensure in one of the following disciplines required:
- LCMHC
- LCSW
- Ability to provide services in various environments and settings
- Availability to work directly with clients and families as needed
- Strong communication, organizational, and clinical skills preferred
