Duties
- Deliver comprehensive personal care including assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor patients’ health by observing vital signs, medication administration, and overall patient condition, ensuring safety and well-being
- Prepare nutritious meals aligned with dietary needs and assist with meal planning and preparation to promote health and comfort
- Maintain detailed care plans and documentation in compliance with HIPAA regulations to ensure privacy and accuracy
- Assist with mobility using equipment like Hoyer lifts or transfer belts, including heavy lifting when necessary, to support patient independence
- Provide companionship and emotional support, engaging clients in activities that promote mental stimulation and social interaction
- Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to foster a clean and organized living space
Experience
- Proven experience providing home & community care or long-term care in settings such as assisted living facilities, nursing homes, or group homes
- Hands-on experience working with individuals with developmental disabilities, autism spectrum disorder, dementia (including Alzheimer’s and memory care), or other cognitive impairments
- Knowledge of patient observation techniques and ability to recognize changes in health status promptly
- Certified in CPR and First Aid; additional certifications like HIPAA compliance are highly valued
- Familiarity with care procedures involving medication administration, vital signs monitoring, and care plan implementation
- Prior work experience in hospice care or home health services is advantageous
- Ability to work independently while managing your own client schedule effectively
