Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or wheelchairs.
- Monitor and observe patients for changes in condition, documenting vital signs and health status accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
- Administer medications following prescribed schedules and medical documentation protocols, ensuring compliance with HIPAA regulations.
- Provide specialized care for clients with dementia, Alzheimer’s disease, or other memory-related conditions, including memory care techniques and behavioral management.
- Support clients with medical procedures such as catheter care, wound dressing changes, and hospice or palliative care needs.
- Prepare nutritious meals tailored to individual dietary requirements and assist with feeding when necessary.
- Maintain a clean and safe environment by performing light housekeeping tasks like laundry, cleaning, and organizing personal spaces.
- Collaborate with healthcare teams to ensure comprehensive patient care, including updating medical records and communicating any concerns promptly.
Skills
- Proven experience in patient observation and home & community care environments.
- Knowledge of medical terminology, medication administration, and vital signs monitoring.
- Familiarity with EMR/EHR systems for accurate documentation of patient information.
- Experience working with individuals with developmental disabilities or long-term care needs.
- Strong understanding of dementia care, Alzheimer’s care, hospice support, and memory care strategies.
- Ability to assist with complex medical procedures such as catheter care and using assistive devices like Hoyer lifts.
- CPR certification and knowledge of safety protocols for patient handling and emergency response.
- Excellent communication skills to effectively interact with clients, families, and healthcare providers.
