Job Description:
Community Support Team Lead (CST) service is a team approach designed to address the identified needs of children and adolescents, who due to serious and chronic symptoms of an emotional, behavioral, and/or substance use disorders, are unable to remain stable in the community without intensive interventions. The Community Support Team Lead (CST) will serve as the primary team leader supporting individuals with Mental Health or Substance Abuse Issues in residential, school, workplace and community settings. He/She will coordinates and monitors the array of services and supports identified in each person’s Person Centered Plan. This service may only be provided to individuals through age 20. This medically necessary service directly addresses the recipient’s mental health and/or substance-related diagnostic and clinical needs.
The Team Leader will provide case management coordination functions including linking, arranging, coordinating and referring consumers to community resources. Additionally, this individual will supervise and lead a team of employees and be required to make independent judgments and use discretion when making decisions related to caseloads.
Minimum Job Requirements:
Education and Experience:
Master’s Degree in human service field and holds a license or provisional license. If provisional, must be licensed within 30 months of hire date
Qualifications:
Must maintain auto liability insurance
Ability to multi-task
Ability to be supervised and willing to comply with different changes in agency’s policy and structure
Ability to make professional decisions for the betterment of the consumers’ treatment
Ability to handle and manage crisis with consumers and consumers’ families
Ability to respond in crisis situations using critical judgment skills
Job Responsibilities: Provide individual and family therapy for each client served by the team
Designate the appropriate team staff such that specialized clinical expertise is applied as clinically indicated for each client
Provide and coordinate the assessment and reassessment of the recipient’s clinical needs
Provide clinical expertise and guidance to the CST team members in the team’s interventions with the recipient
Provide the clinical supervision of all members of the team for the provision of this service
Oversight the responsibilities of the Qualified Professional (QP) and assist the QP with responsibilities outlined in the QP job description
Direct and assist with the responsibilities of all other CST staff as outlined in their job description including participating in the person centered planning process and crisis interventions
Job Type: Full-time
Pay: $60,000.00 - $70,000.00 per year
