Responsibilities
Assist resident with all personal care skills (bathing, dressing, grooming, toileting) and services in accordance with the Individual Resident Service Plan, using safe, courteous, and effective standards of practice.
Make appropriate observations about the resident's condition during interaction with the resident and record changes in a timely manner. Report any changes to the H&W Director, Nurse, or designated charge person immediately.
Records omission of resident services/medications according to company policy/procedures, and standards.
Assists in recording temperature, blood pressure, pulse, and respiration rates, and food and fluid intake and output, as directed.
Aid in the delivery of specific participation in programs, such as fall prevention, safety, and socialization to all residents.
Participate in family conferences as needed to discuss the resident's status and review the appropriate service plan and medication regimen.
