What are the responsibilities and job description for the Licensed Practicing Nurse (LPN) Administrative position at Stellar Care?
This is a part time administrative nursing position. 3 days/20-24 hours per week in office. Preferred days can be discussed during the interview and can be performed Monday through Thursday.
Licensed Nursing professional, as the name suggests works with program professionals to assure the health of individuals served are being met in accordance with acceptable medical practices, while adhering to and assuring leadership stays in compliance with applicable Medicaid and State rules, laws, and regulations. In this case, specifically the HCBS-DD Medicaid waiver for adults with Intellectual and Developmental Disabilities receiving comprehensive services and supports through Stellar Care and Services, LLC. The nursing professional must take all efforts to assure good working relations with all stakeholders, including but not limited to employees, contractors, individuals served, families/guardians, IDT members and other community members associated with Stellar Care or the individual served. This position is NOT a bedside nurse; instead, is a administrative support nurse utilized for chart auditing, developing medical protocols, QMAP training, self-medication assessments, other medical training, etc.

QUALIFICATIONS:
A Nursing Professional must possess the following academic qualifications: A license to work as an LPN or above in the State of Colorado by the Department of Regulatory Agencies (DORA); certification through DORA must remain in good standing and updated annually as required by Colorado law prior to performing nursing duties. Must be able to register with the State Division for Developmental Disabilities and obtain required credentials necessary to teach employees and contractors medication administration certification (QMAP). Prior experience and expertise in nursing. Prior experience working with individuals who have a variety of intellectual and developmental disabilities is PREFERRED BUT NOT REQUIRED. An ability to use basic computer related programs including but not limited to Microsoft office applications. Knowledge of rules and regulations associated with medical supports of individuals with intellectual and developmental disabilities under the HCBS-DD Medicaid Waiver.
DRIVING STATUS:
This position is designated as an unlimited driving position, though driving will be minimal. If it is agreed upon that the LPN becomes an employee in this position then the employee must comply with the requirements outlined in Stellar Care POLICY AND PROCEDURES. All employees in this position must have a current, valid, U.S. driver’s license with acceptable status, as defined by our insurance carrier. An employee hired for a position that requires the employee to drive his/her own car is responsible for his/her own automobile insurance. Mileage is reimbursed.
DUTIES:
- Works with the person served team and physicians to develop and manage care plans
- Instruct patients and families in proper treatment as necessary
- Helps patients take steps to improve or maintain health
- Responds to inquiries in a responsive and professional manner
- Performs / conducts assessments of individuals served ability to self-medicate.
- Develops individualized protocols pertaining to seizures, feeding, breathing, choking, G/J-tube and any other medical related support need for the person served.
- Provides qualified medication administration training (QMAP) to all employees and contractors. Develop a class in each office monthly and assure certificates are turned into HR timely.
- Monitor and audit medical charts quarterly and report discrepancies to the program managers/director as needed.
- Assist program director/coordinator with audit corrections.
- Conduct G-Tube training and Semi-annual G/J-tube observations with providers and assure documentation is submitted timely to program managers and director.
- Report all critical and non-critical incidents to direct supervisor immediately, and write Incident Reports within 24 hours.
- Must have the ability to speak and write the English language legibly and be able to read English language appropriately when needed.
- Must be able to lift and transfer up to 50 lbs.
