Responsibilities
The essential function of this position is to provide hospice nursing and palliative care to terminally ill patients in their home. The hospice nurse will ensure the highest level of care is provided to our patients as they transition to end-of- life care. Professionalism and compassion are essential to success in this role.
- Assess and admit patients to home hospice programs.
- Develop plan of care in collaboration with patients, families, physicians and members of the care team
- Provide new patients with information on home care hospice services and provide initial patient education
- Maintain proper documentation in the patient health record to maintain regulatory compliance
- Provide both physical and emotional support to patients by honoring the patient and family with compassion and care at this pivotal time.

Requirements
• RN or LPN licensure in the State of Maryland
• Current Cardio Pulmonary Resuscitation (CPR) certification
• Prior experience in hospice or home health strongly required
• Valid driver’s license and reliable transportation
• Certified Hospice and Palliative Nurse (CHPN) a plus
• Commitment to Stella Maris Hospice’s philosophy of care, values, mission statement and organizational culture
• Willingness to serve on-call
