Position Summary
As the QIDP, you will be responsible for coordinating the admission of individuals to the Intensive Treatment Program (ITP) as well as providing liaison services between NWC, guardian/family and community agencies. You will also assist with case management responsibilities as needed to include functional assessments and development of individual support plans, monitoring and integrating client programs to ensure continuous active treatment and monitoring all aspects of the clients Individual Support Plan.
Salary Information
The starting pay is between $40.00 to $41.47 per hour (approximately $83,200 to $86,800 annually), depending on qualifications plus excellent benefits. A 12-month probationary period is required.
Pay for current or former state employees will be set based on the rules that apply to compensation for the applicable transaction. This position is classified as a Developmental Disabilities Coordinator and is in pay schedule and range 11/03.

All applicants will be required to allow DHS to conduct a background check to determine whether the circumstances of any convictions may be related to the job being filled.
All appointees will be required to receive a TB test, which will be administered by NWC or medical personnel approved by the facility prior to their start date.

Qualifications

- A professional who has at least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities AND
- Is one of the following: (1) a doctor of medicine or osteopathy; (2) a registered nurse; or (3) an individual who holds at least a bachelor's degree in a professional category specified in the Federal Register as meeting the QIDP qualifications as outlined in 483.430 of the Code of Federal Regulations . Note: Official transcripts will be required prior to appointment to the position.
- Experience serving as a liaison with external partners, performing duties such as obtaining consents, informing guardians of significant changes with status/injuries, facilitating legal/court interactions, etc.
- Experience developing and implementing care plans for persons with a disability, including the design of goals and objectives.
- Experience working with persons with intellectual disabilities with co-occurring mental health and behavioral disorders.
- Experience taking referral calls and maintaining a referral database .
- Experience providing Case Management services for persons with Developmental Disabilities in a residential treatment setting.
- Experience leading a team including both internal and external partners to provide services to individuals with Developmental Disabilities.
