Introduction
ABOUT THE HUMAN SERVICES DEPARTMENT OF JEFFERSON COUNTY Jefferson County Human Services is home to the Children’s Long Term Support (CLTS) Waiver Program that provides structure within which Medicaid funding is available. This funding supports children and youth who live at home or in the community and have substantial limitations in multiple daily activities as a result of one or more of the following: Intellectual and/or developmental disabilities, Physical disabilities, and or Mental health disabilities.
Position Summary
WHAT YOU WILL DO To meet the goals of the program, the team needs to add a Children’s Long-Term Support (CLTS) Support and Services Coordinator that will provide assistance and support to children with disabilities and their families, so they can maintain optimal quality of life within their own home and community.
SOME ESSENTIAL RESPONSIBILITIES You will be successful in this role by completing the following tasks and responsibilities:
- Providing assessment, case planning, protections, and case management services.
- Providing authorizations for each CLTS Waiver billable service.
- Coordinating authorization from third party administrators for necessary services.
- Implementing comprehensive service plans to meet the individual needs and outcomes.
- Referring clients to community resources and facilitates ongoing support services.
- Variety of other necessary tasks listed on the post job description.
Salary Information
We pay $28.18 - $29.80 per hour based on candidate’s successful experience and qualifications. Potential to earn up to $37.09 per hour.

Qualifications
Requirements:
- Bachelor’s degree in social work or related field plus two years of experience; or an equivalent combination of education and experience
Preferred Requirements:
- Bachelor’s degree in social work or closely related field plus three years’ experience.
- State of WI Social Worker Certification.
- Certifications/Licensures
- CLTS Waiver Certification required within 30 days of hire
- Valid Driver’s License
