Introduction
Sand Ridge Secure Treatment Center (SRSTC) within the Department of Health Services (DHS) is hiring two full-time Patient Records Associates. Work hours are 7:45 a.m. to 4:30 p.m., Monday through Friday, with weekends off.
SRSTC opened in 2001 and provides care and treatment for several different patient populations. The center houses Wisconsin’s Sexually Violent Persons Program, provides care and treatment for persons found not guilty by reasons of mental disease or mental defect, and provides court ordered treatment, testing, and education to patients found not competent to stand trial. SRSTC is a 500-bed treatment facility and is part of the Division of Care and Treatment Services (DCTS). Watch this video to learn more about the work of DCTS.

Position Summary
As a Patient Records Associate at SRSTC, you will be responsible for the maintenance, security, and flow of records, documents, forms, and patient files through the records unit and to other internal and external partners throughout the facility. You will:
- Process court orders, admission, transfer, and discharge information according to Chapter 980.
- Provide assistance to patients, institution staff, attorneys, law enforcement agencies, courts, other agencies, management staff, and the public regarding procedures related to patient movements, visits, and legal document copy requests.
- Schedule all patient professional visits and telephone calls.
- Process the patient annual re-exam paperwork and reports.
- Coordinate the release of patient information.
- Maintain active and inactive patient permanent charts.
- Review treatment records for accuracy and completeness.
- Adhere to all federal and state regulations regarding privacy and confidentiality of all patient information including the Health Information Portability and Accountability Act (HIPAA).
Salary Information
The starting pay is $22.76 per hour, plus excellent benefits. A 12-month probationary period is required.
Pay for current or former state employees will be set based on the rules that apply to compensation for the applicable transaction. These positions are classified as Offender Records Associates and are in pay schedule and range 02/11.
Job Details
All applicants who may be appointed to this position will be required to allow DHS to conduct a background check to determine whether the circumstances of any conviction may be related to the job being filled.
All appointees will be required to receive a TB test, which will be administered by SRSTC or medical personnel approved by the facility after to their start date.
DHS does not sponsor work visas for this position, thus, in compliance with federal law, selected candidates will be required to verify eligibility to work in the United States by completing the required I-9 form upon hire. DHS is not an E-Verify employer nor a STEM-OPT eligible employer.
Qualifications
Minimally qualified applicants will have all of the following:
- Experience using a variety of computer programs (e.g., Word, Excel, Access, Outlook, Electronic Health Records, etc.), to perform duties such as creating memos, emails, and spreadsheets.
- Experience providing customer service/support (e.g., directing phone calls, gathering information, answering questions, providing instruction, serving as a liaison, resolving customer complaints/conflict, etc.).
- Experience maintaining a record system (e.g., data collection, data entry, maintaining a filing or tracking system, providing data or records in response to requests, etc.).
- Experience scheduling and coordinating appointments with a variety of relevant parties.
- Ability to type at least 45 words per minute (A typing test will be required).
Well-qualified applicants will also have one or more of the following:
- Experience working in an environment that handles sensitive or confidential information such as finance, healthcare, government, etc.
- Experience responding to requests for information using written and oral communication.
- Experience working in a secure setting.
