The Utah Division of Services for People with Disabilities (DSPD), within the Utah Department of Health & Human Services (DHHS), is looking for a dedicated professional to fill our Caseworker Specialist II position. As a Caseworker Specialist II, you will assist client families and people with disabilities to make informed choices, coordinate waiver services, and use person-centered planning to ensure services meet the needs and desires of the individuals. You will also be be conducting family meetings, training sessions, and will monitor budget expenditures.

Roles And Responsibilities
- Refer clients and families to services offered to clients on the Waiver as well as community resources.
- Travel to meet with clients and families throughout the State of Utah.
- Ensure SAS paperwork, case histories, activity logs, social summaries, needs assessments, service plans, and correspondence are completed and current.
- Provide clients with information concerning rights, options, benefits, services, goals and expectations.
- Interact with the family and/or client/resident to discuss need and available services
- Develop and implement waiting list plans to meet those needs.
- Monitor budget expenditures.
- Provide consultation, make recommendations, give appropriate advice, and/or facilitate decisions.
- Has knowledge of Home and Community Based Services (HCBS).
- Has a Bachelor's Degree in a human services related field or 5 years of relevant experience.
- Has at least one year of experience working directly with persons with intellectual disabilities, other developmental disabilities, or acquired brain injuries.
- Has strong interpersonal skills.
- Is passionate about empowering People with Disabilities.
- Is an excellent listener who is also very proactive
- Is bilingual in English and Spanish.
- The typical office setting for this job is adequately lighted, heated and ventilated. However, you may encounter some risks that are associated with any office setting, for example, the safe use of office equipment, avoiding trips and falls, observing fire regulations, etc.
- DHRM rules regarding promotions and transfers apply to current State of Utah employees.
- This position requires in-state travel with the ability to use a state registered vehicle.
