The Division of Family Health is looking for an Intensive Care Coordinator that is experienced in casework to join their diverse team to help ensure youth and families in the Carbon and Emery County communities are receiving the proper support to lead safe and healthy lives.

Intensive Care Coordinator

FT / PT Status – Full-Time Salary – $20.89 - $31.41 Hourly Wage Remote Work / In-Office – In-Office / On-Site

Schedule

The schedule for this position is typically 10 hours per day Monday - Thursday

Key Responsibilities

Key responsibilities and day to day responsibilities of this position:

  • Facilitate case management services using the High Fidelity Wraparound Model
  • Practice independent judgement and discretion while carrying a caseload; caseload is based upon the expectation of meeting with children, youth, young adults and families face to face 50% of the 40 hour work week
  • Evaluate the needs of the child, youth, young adult and family and provide in person support, skills, coordination, assessment, and intervention
  • Build strong rapport and relationships with children, youth, young adults, caregivers and professionals
  • Develop wraparound plan with families and professionals by creating goals with measurable outcomes and time bound specific objectives; develop & maintain a crisis prevention plan throughout the process with each child, youth, young adult and family
  • Complete assessments program documentation with child, youth, young adult and family to support them in stabilizing in their communities while reaching their goals
  • Connect with professionals and evidenced based interventions to meet the mental health/ behavioral health needs of the child, youth, young adult and family
  • Show innovation and creativity in developing individualized plans that allow for a child, youth, young adult and family to be successful in their community
  • Maintain documentation in the electronic health record and meet all documentation deadlines
  • Attend trainings with a goal to grow and develop case management skills
  • Attend staff meetings and work cohesively with your team in the office

Qualifications

Minimum or "day-one" qualifications:

  • Demonstrate flexibility and ability to meet youth/families without judgement or criticism
  • The ability to manage your own emotions during times of crisis or other’s dysregulation
  • The ability to manage a schedule that is primarily in the field but includes some office work
  • A desire to be in the field working directly with families and partners
  • Desire to actively look for ways to support others in reaching their goals
  • Problem solving skills and overall positive outlook
  • The ability to communicate information and ideas in speaking so others will understand.
  • Giving full attention to others to understand, asking questions as appropriate.
  • Talking to others to convey information effectively.
  • Being aware of others' reactions and understanding why they react as they do.
  • Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance.
  • The ability to read and understand information and ideas presented in writing.
  • The ability to communicate information and ideas in writing.
  • The ability to recognize a problem.

Preferred Or Trainable/on-the-job Qualifications

  • Bachelor Degree in Social Work, Social Services, or Behavioral Science.
