Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and administer medications according to personalized care plans.
- Provide wound care, tube feeding management, airway management which includes nebulizer treatments, CPAP Therapy, CPT Vest Therapy, Oxygen Therapy as needed, and suctioning support as needed for patient.
- Document all patient interactions accurately
- Coordinate with physicians, case managers, families, and caregivers to ensure seamless continuity of care across settings.
- Educate patients and their families on medication administration, disease management, nutrition, and safety measures to promote the client's well-being.
Qualifications
- Valid LVN license in the state of Texas with current CPR certification.
- Recent graduate or limited Home Health Experience will be considered but proven experience working with individuals in specialized settings such as assisted living facilities or skilled nursing facilities is a plus.
- Excellent communication skills for caregiver collaboration and Medical Team are essential.

Pay: $26.00 - $30.00 per hour
Expected hours: 38.0 – 40.0 per week
