This is first-level supervisory support care work involving professional program planning and management for a treatment sub-unit or multiple residential locations. The incumbent supervises direct care staff and assists with the development, implementation, coordination, and evaluation of individual habilitation or treatment plans for individuals with developmental disabilities. Work includes coordinating daily support care activities, reviewing documentation, monitoring service delivery, and ensuring assigned staff follow applicable policies, procedures, and care requirements.
Examples of work include supervising direct care staff assigned to a treatment sub-unit or residential location; assisting with the development, implementation, coordination, and evaluation of individual habilitation or treatment plans; monitoring resident care activities and reporting changes in resident condition or behavior; reviewing care documentation, records, and reports for accuracy and completeness; assigning and coordinating work activities for direct care staff; training, coaching, and providing guidance to staff; assisting residents with activities of daily living as needed; supporting rehabilitation, ambulation, transfers, and other care-related activities; communicating with professional staff, supervisors, residents, and families as appropriate; ensuring compliance with agency policies, safety practices, and treatment requirements; and performing related duties as assigned.
Typically requires a high school diploma or high school equivalency and 3-5 years of experience. May require Certified Nurse Assistant (CNA) designation.
