Main Purpose of Job

This position is a non-medical case manager providing comprehensive non-medical case management services to individuals living with HIV/AIDS within the local community for the Frederick County Health Department. This position will assist clients with accessing supportive services, reducing barriers to care, improving retention in medical services, and promoting overall stability and self-sufficiency. This position will work collaboratively with healthcare providers, community agencies, and team members to coordinate client centered services in accordance with the Ryan White HIV/AIDS Program standards and Maryland Department of Health requirements.

MINIMUM QUALIFICATIONS

Education:� �Graduation from an accredited high school or possession of a high school equivalency certificate. Experience: One year of administrative staff or professional work in a health care setting. Notes: 1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience. 2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.
