Description

Glasgow State Nursing Facility (GSNF) may be the place for you. We are a state-of-the-art 100-bed long-term care nursing facility for individuals with mental illness or intellectual disabilities. We are seeking a Social Service Clinician I to join our team in caring and improving the daily lives of our residents.
Responsibilities include, but are not limited to:

Responsibilities include, but are not limited to:
- Provides intensive and therapeutic social work through the assessment of resident needs and the provision of social services to complex cases at Glasgow State Nursing Facility.
- Provides therapeutic counseling, advocacy, and case coordination of individuals with mental illness and developmental disabilities in a long-term care facility.
- Coordinates placement of residents into the community in partnership with Comp Care Center, KY Transitions, State Guardianship, and Support for Community Living providers.
- Coordinates referrals to other facilities and completes appropriate discharge paperwork per policy.
- Assists with admissions, including scheduling and completing appropriate paperwork and distributing to applicable personnel.
- Completes individualized psychosocial assessments and plans of care for new residents and quarterly for all other residents.
- Enters data in computer systems assuring compliance with facility, state, and federal mandated regulation, policies, and procedures.
- Participates in interdisciplinary committee meetings and serves as an advocate for the resident.
- Collaborates with the activity department to ensure therapeutic activities are conducted as described in the resident’s individual plan of care.
- Participates on committees as assigned.
- Contacts resident representatives regarding change in condition as needed.
- Performs other duties as assigned.

Minimum Requirements
EDUCATION: A master's degree in social work, sociology, psychology, marriage and family therapy or a related field.
EXPERIENCE, TRAINING, OR SKILLS: Must have one year of professional social work experience.
Substitute EDUCATION for EXPERIENCE: NONE
Substitute EXPERIENCE for EDUCATION: A bachelor's degree supplemented by three years of professional social work experience will substitute for the primary education and experience requirements.
SPECIAL REQUIREMENTS (AGE, LICENSURE, REGULATION, ETC.): If employed by the Cabinet for Health and Family Services or the Department of Juvenile Justice, an employee in this job classification must furnish to the appointing authority within one month of employment, documentation that he/she possesses a valid driver's license. Must maintain any required licensure(s), certification(s), or other credentials for the length of employment in this job classification. Employing agency is responsible for ensuring employee possesses and maintains required licensure(s), certification(s) or other credentials.
Working Conditions
Performs job duties in an office/facility setting and visits clients and/or families at their residence. Provides emergency services on 24-hour basis, including accepting referrals and completing investigations at any hour without prior warning. As a result of enforced intervention, the worker runs the risk of physical harm.
Probationary Period
This job has an initial and promotional probationary period of 6 months, except as provided in KRS 18A.111.
