Position Overview

The Division of Behavioral Health and Recovery is seeking to hire a Social Worker II for the Elizabeth Packard Mental Health Center located in Springfield, Illinois to serve as member of a multi-disciplinary treatment team and performs extensive casework in the role of Treatment Coordinator. Provides comprehensive social work services for the unit including formulation of psychosocial assessments. Designs, develops, implements, and evaluates treatment plans. Provides individual, group, and family therapies and develops discharge plans for continuing recovery in the community. Serves as a member of various Performance Improvement Teams to promote quality and safety initiatives. Develops comprehensive psychosocial assessments with information gathered through interviews. Assesses financial status of assigned patients.

Essential Functions
Serves as member of a multi-disciplinary treatment team and performs extensive casework in the role of Treatment Coordinator.
Provides individual therapy and counseling to assigned patients.
Develops comprehensive psychosocial assessments with information gathered through interviews with patients, families, and community referral sources.
Provides discharge planning to link individuals with appropriate services for continued recovery in the community.
Assesses financial status of assigned patients.
Serves on the facility hospital performance improvement teams to promote quality and safety initiatives.
Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
Minimum Qualifications
Requires a master’s degree in social work from a recognized college or university supplemented by one (1) year of professional social work experience in the evaluation and treatment/habilitation of mentally ill, developmentally disabled or other disabled persons.

Conditions of Employment
Requires the ability to physically restrain patients as necessary to prevent injury to patient and others.
Requires the ability to obtain and maintain Cardiopulmonary Resuscitation (CPR) certificate.
Requires the ability to stoop, bend, lift or exert up to 20 pounds of force in performance of duties.
Requires the ability to physically access various worksites throughout the hospital for administrative and clinical duties.
Requires the ability to work after business hours, weekends and holidays.
Requires ability to pass the IDHS background check.
Requires ability to pass a drug screen for drugs prohibited from recreational use under Illinois Law.
