The Department of Economic Security, Division of Developmental Disabilities (DDD) is seeking an experienced and highly motivated individual to join our team as a Case Manager Lead – Early Intervention/Community. The position serves persons with developmental disabilities by supporting them and their families to lead the healthiest, most self-sufficient, productive lives within their capabilities. Gathers all necessary information and documentation for determining eligibility and/or re-determining eligibility for Division of Developmental Disabilities (DDD) services following federal and state mandates, agency policy and procedures relating to Early Intervention, educational programs, behavioral health, vocational rehabilitation services, Arizona Health Care Cost Containment System (AHCCCS) long-term care, targeted support coordination and state contracted or non-contracted agencies.
• Conduct on-site home visits to meet with/member/families to develop relationships & become familiar with/family dynamics/needs & makes appropriate referrals to community agencies.
• Facilitates multi-disciplinary Individualized Family Support Plan (IFSP) meetings for member/children (minimum of every 6 months on every child) to determine & identify services for member. Works with/Department. of Child Safety to meet child's need while in the system.
• Researches community for resources to meet needs identified in plan, informs member/family of options. Assists family with enrollment into DDD Early Intervention Program & secure identified/needed services for all children/member.
• Writes comprehensive progress notes and maintains case records. Prepares a variety of reports and completes special projects. Develops and maintains files for each child. Attends staffing meetings, training and other meetings (including statewide travel).
• Knowledge of procedures, and techniques used in the development of Individualized Family Service Plans (IFSP)
• Knowledge of Health Insurance Portability and Accountability Act (HIPAA),
• Knowledge of social emotional, health, and/or educational needs of children
• Skills in oral and written communication
• Skills in Conducting needs assessments
• Skills in interviewing, counseling, and interacting with members and family members
• Skills in problem identification and resolution
• Ability to work independently and within diverse work teams and maintain confidentiality
• Ability to lead a team of Support Coordinators in the absence of the supervisor
• Ability to organize and manage case load

• Candidate must have a Bachelor’s degree in Early Childhood, Early Child Special Education, Family Studies, OR a Bachelor’s degree in a closely related field (e.g., Psychology, Social Work, Elementary Education, or Sociology) with review of course and approval by ADES/AzEIP; OR if hired in the AzEIP system as a Service Coordinator before July 1 2001, documentation of continued satisfactory work in the AzEIP system in a Service Coordinator position from that date of hire.
• Arizona Level 1 Clearance Card is required
• Requires a valid state-issued driver’s license for In-state travel
• Successfully pass background and reference check; employment is contingent upon completion of the above-mentioned process and the Agency’s ability to reasonably accommodate any restrictions
