Job Title: Part-Time Nurse Practitioner – Primary Care (Assisted Living)
Schedule: 20 Hours per Week
We are seeking a compassionate Nurse Practitioner (NP) to provide primary care services to residents in an assisted living community. This part-time role offers the opportunity to build meaningful relationships with residents while delivering high-quality, patient-centered care in a supportive environment.
Responsibilities include:
- Conduct routine medical visits and health assessments for residents
- Diagnose and manage acute and chronic conditions
- Prescribe medications and treatments as appropriate
- Develop and oversee individualized care plans
- Collaborate with nursing staff, caregivers, families, and outside providers
- Maintain accurate and timely documentation in the EMR
Qualifications:
- Master’s or Doctoral degree from an accredited Nurse Practitioner program
- Current NP license in the state of practice
- Board certification (FNP or Adult-Gerontology Primary Care NP preferred)
- DEA license and prescriptive authority (if required by state)
- Experience in geriatrics, primary care, or long-term care preferred
