Job Title: Peer Support Specialist Program Supervisor

Reports To: Licensed Professional

Education Experience:

Peer Support Specialist Program Supervisor serving all members must have the following minimum qualifications:

    • Meet North Carolina’s definition of a Qualified Professional per 10A-NCAC 27G.0104; and
    • A bachelor’s degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area, or licensure as a registered nurse (RN); and
    • Two years of experience working directly with members with behavioral health conditions (if serving members with behavioral health needs) or with an I/DD or a TBI (if serving members with I/DD or TBI needs).

Main Function:

The primary responsibilities of the Peer Support Specialist Program Supervisor are to promote whole person, integrated care by planning, coordinating, tracking, closing of gaps in care and monitoring care to members to achieve the key goals of member’s person centered plan.

General Duties/Responsibilities:

  • 1. Trained in quality supervisory skills through ab approved CPSS supervisory training program.

  • 2. Complete the CPSS Certification program and possesses knowledge of the CPSS role and work, as well as understand the principles and philosophy of recovery and the code of ethics of the NC Peer Support Specialist Certification Program.

  • 3. Complete mandatory CPSS supervisor training within 90 days of hire.

  • 4. Understand and support the role of the CPSS.

  • 5. Understand and promote the beneficiary’s recovery.

  • 6. Advocate for the CPSS and PSS across the organization and in the community.

  • 7. Promote both the professional and personal growth of the CPSS within established human resource standards.

  • 8. Coordinate assessments needed for the beneficiary.

  • 9. If appropriately licensed, the QP may conduct the assessments.

  • 10. Collaborate with beneficiary(s) and CPSS to develop recovery-oriented person-centered plan for the beneficiary that demonstrates consideration for integrated care.

  • 11. Conduct at least one in-person, telehealth, or telephonic, audio-only communication contact with the beneficiary within 90 days of PSS being initiated and no less that every 90 days thereafter to monitor the beneficiary’s progress and effectiveness of the program; and to review with the beneficiary the goals of their PCP and document progress.

  • 12. Plan work assignments, monitors, reviews, and evaluates work performance of program staff and facilitates staff meetings and conduct routine reviews of service notes for quality assurance.

  • 13. Provide administrative and supportive supervision to program staff individually at least once per month or more if needed. Provision of supervision must be based on the experience of the individual staff. If on leave, obtain adequate supervision coverage for supervised staff.

  • 14. Collaborate with program staff to assess strengths and areas of growth and develop an individual supervision plan.

  • 15. Collaborate and foster collegial roles with program staff.

  • 16. Determine team caseload size based on the level of acuity and needs of the beneficiary(s).

  • 17. Facilitate or co-facilitate skill building recovery groups based on the needs or request of beneficiaries.

  • 18. Ensure referrals for community resources requested by beneficiary(s) are completed.

  • 19. Coordinate Care Transition: Coordinates follow-up services for members with recent inpatient hospitalization or Emergency Department visit within 24 hours of discharge. Coordinates after care needs for transitions in care such as release from incarceration, change in housing, or other life transitions.

  • 20. 24:7 Call / Crisis Response Coordination:Identifies and provides emergency crisis response coordination as necessary and following agency policies related to crisis. Participation in agency on-call structure may be required and in 24:7 crisis response for members. Participates in post crisis team debriefing and provides feedback on ways to prevent future crisis for the member served.

  • 21. Establish Relationships with Integrated Care / Team and Community Resources:Communicates effectively with members receiving services, providers, and other natural supports as needed. Provide education regarding services to all parties involved in the care and support of the member. Establishes collaborative relationships with integrated care team members and community resources within the assigned geographical region to improve resource linkage.

  • 22. Maintain Documentation: Maintain medical record compliance/quality. Ensure timely documentation of care coordination activities as required by department policy and procedures.

  • 23. Other Duties as assigned including but not limited to: Review data to identify and determine appropriateness for services, which includes monitoring utilization, reporting, clinical measurement data and compliance issues. Demonstrate knowledge of and comply with all agency policies and procedures, as well as service definitions related to specific program areas. Maintain trainings as required and requested. Participates in Quality Improvement (QI) projects, to improve service delivery, costs of care, on an as needed basis. Complete all other relevant responsibilities as assigned by the supervisor. Driving and travel may be required.

  • 24. Establishes a caring, therapeutic relationship with member.

  • 25. Demonstrates understanding of members rights and ensures member’s rights are protected.

  • 26. Maintain a valid North Carolina Driver’s License, acceptable criminal record and Health Registry Check that adhere to standard of employment.

  • 27. Demonstrates knowledge of START Wellness, Inc. policies and procedures defined in the employee handbook.

  • 28. Demonstrate professional work ethics.

  • 29. Complete training requirements. Attends all training sessions as required by the agency.

  • 30. Participates in activities to promote personal growth and development.

  • 31. All other duties as deemed necessary by START Wellness, Inc.
