What You Bring To SBHG
- Master's degree in social work, marriage & family therapy, or psychology from an accredited college or university required.
- Licensed as AMFT, ACSW, APCC, LMFT, LCSW, or LPCC required.
- (1) year direct service experience with emotionally disturbed adolescents in a healthcare setting required (student internship and/or practicum time may be substituted).
- Experience working in an inpatient acute setting preferred.
- Prepares a comprehensive written psychosocial evaluation and assessment for admitted clients.
- Participates in treatment team meetings and is responsible for a coordinated treatment plan for each client.
- Provides individual and family psychotherapy.
- Responsible for permanency planning including assessing client's readiness for discharge; coordinating and planning with treatment team; making appropriate referrals for placement, case management and aftercare services; and coordinating benefits with Business Office personnel.
- Prepares and submits client documentation in EMR according to specified timelines.
- Provides consultation as needed with families on issues regarding admission, discharge, and aftercare plans.
- Coordinates client intake, treatment, and discharge actions with county partners.
- 24/7 inpatient involuntary program
- 16-bed facility for acute youth (ages 5-17)
- Intensive treatment programs with individualized care plans
- Available for contracting Central Valley counties
- Moves clients through five phases: admission; engagement; learning; stabilization and linkage to resources and aftercare
