Position Overview

The Medical Records Clerk supports a litigation-focused law firm by obtaining, organizing, tracking, and maintaining medical records and related documentation. This role helps ensure attorneys, paralegals, and legal nurse consultants have timely access to accurate, complete, and well-organized medical documentation for discovery, depositions, expert review, and trial preparation.

Reports To: Supervising Attorney, Paralegal, or Legal Nurse Consultant

FLSA Status: Non-Exempt

Location: Birmingham, Alabama

Key Responsibilities

· Request, obtain, and track medical records and itemized bills using HIPAA-compliant authorizations, provider portals, vendors, subpoenas, and court orders; maintain accurate logs for requests, correspondence, invoices, and deadlines.

· Organize and maintain records across multiple cases, including scanning, OCR processing, electronic filing, storage, retrieval, quality control, Bates labeling, naming conventions, metadata, and patient/admission verification.

· Review and summarize records for attorneys, identifying treatment history, diagnostic information, documentation gaps, chart deficiencies, and issues requiring follow-up.

· Coordinate with attorneys, paralegals, legal nurse consultants, and vendors to provide status updates and support discovery, expert review, depositions, and trial preparation.

· Maintain strict compliance with HIPAA, applicable privacy rules, Business Associate Agreement terms, confidentiality requirements, and the “minimum necessary” standard.

· Accurately document time spent performing billable case-related work, including reviewing and summarizing medical records, for client billing purposes.

Required Qualifications

· High school diploma or equivalent required; HIM, healthcare administration, paralegal studies, or related postsecondary training preferred.

· Minimum of three (3) years of medical records experience in a healthcare, legal, insurance, or release-of-information setting.

· Working knowledge of medical terminology, HIPAA, release-of-information practices, confidentiality requirements, and common medical record components.

· Proficiency with EMR/EHR systems, provider portals, law firm case management software, Microsoft Office, Adobe Acrobat, scanning, OCR, and electronic document management tools.

· Ability to identify missing records, incomplete productions, duplicate records, documentation gaps, and chart deficiencies requiring follow-up.

· Strong organizational skills, attention to detail, written and verbal communication skills, discretion, and ability to manage multiple cases, priorities, and deadlines in a litigation environment.

Compensation

Salary Range: $40,000 - $55,000.
