Position Summary
The Nurse Family Partnership (NFP) Nurse Home Visitor is responsible for providing comprehensive
nursing services to women and their families eligible for the NFP Program. Nurse Home Visitors (NHV) are
pivotal in transforming the lives of vulnerable babies, mothers, and families by implementing and maintaining the fidelity of the NFP model. Our goal is that all children are healthy, families thrive, communities prosper, and the cycle of poverty is broken. Nurse Home Visitors report to the NFP Nurse Supervisor.

- Provides home visits to women and their families eligible for the NFP Program.
- Adheres to nursing process and the NFP model of home visitation.
- Carries a caseload of up to 25 clients.
- Written and Verbal Communication- NHVs need to communicate effectively, openly, and consistently with their families, colleagues, and stakeholders.
- Bi-lingual/Spanish and/or Arabic language proficiency highly desirable.
- Current RN license (in good standing) required.
- Current CPR required.
- Adheres to all Public Health Standards of practices
- Time Management- NHVs who can flexibly schedule and reschedule a day’s activities based on multiple factors.
- Ability to work autonomously
- Listening- interactions between NHVs and clients depend on the NHVs ability to listen attentively and accurately to the client’s current experience.
- Ability to impose structure- in between and during home visits, NHVs must be able to create structure in moments of confusion, disorganization, or crisis.
- Excellent written and verbal communication skills.
- Basic computer skills.
- Willingness to provide Home Visiting services
- Able to attend education sessions.
- Valid driver’s license and / or reliable transportation.
