Position Summary:
The Registered Nurse provides skilled nursing services to home health patients, including assessments, care planning, wound care, medication management, and coordination of patient care with the interdisciplinary team.
Key Responsibilities:
- Perform comprehensive patient assessments and develop individualized care plans
- Provide skilled nursing services such as wound care, injections, IV therapy, and medication management
- Educate patients and caregivers on disease management, home safety, and care instructions
- Prepare accurate and timely clinical documentation (OASIS, visit notes, recerts, discharges)
- Coordinate patient care with physicians, therapists, and other team members
- Monitor patient progress and update care plans as needed
- Participate in case conferences and interdisciplinary meetings
- Ensure compliance with Medicare guidelines, state regulations, and agency policies
Qualifications:
- Active Arizona Registered Nurse (RN) license – Required
- Minimum 1 year of nursing experience (home health preferred)
- OASIS experience a plus but willing to train the right candidate
- Strong clinical assessment and communication skills
- Ability to work independently and manage a flexible caseload
- Reliable transportation and valid driver’s license
