Summary
We are seeking a compassionate and dedicated Case Manager to join our team at Star Choices, Inc. In this role, you will provide personalized support and guidance to assist individuals with disabilities.
Responsibilities
- Assist in accessing community resources and services.
- Monitor progress and adjust plans as necessary to ensure optimal outcomes.
- Advocate for the rights and needs of people supported.
- Maintain accurate documentation of services provided and client interactions.
- Oversight of Private Home Care.
- Provide phone duty relief.
- Participate in weekly team meetings.
- Participate in monthly DSP meetings.
- Provide direct support as needed.
Requirements
- High School diploma or GED required, Bachelor's degree strongly preferred.
- At least 1 year of experience working with individuals with intellectual and developmental disabilities is highly desirable.
- Must also be able to learn relevant software applications, including electronic medical records
- Dependable Vehicle
- Valid Georgia Driver's License and good driving record
- Current Auto Insurance
- Acceptable drug screen and background check.

Pay: $18.00 - $20.50 per hour
Expected hours: 40 per week
