What are the responsibilities and job description for the Physician -Internal Medicine -LTC & SNF - Flexible schedule 1-3 days position at Star Choice Health?
Job Summary: We are currently seeking skilled Internal Medicine Physicians to provide services in our sub-acute and long term care settings in Maryland.
Tasks include but not limited to:
-Must be an excellent communicator with outstanding interpersonal skills, including written and verbal communication.
- Collaboration with other healthcare professionals to develop and implement effective patient care plans.
- Diagnose and treat acute and chronic illnesses, injuries, and conditions.
- Prescribe medications and other treatments.
- Perform evaluations on patients with changes in condition upon facilities request .
- Order and interpret diagnostic tests, such as laboratory and imaging studies.
- Adhere to infection control protocols to ensure patient safety.
- Document promptly all patient encounters and maintain accurate medical records using electronic health record systems (e.g., GEHRimed and PCC).
- Attend to patients and their loved ones, providing information and education about preventive care, health promotion, and disease management as needed.
-Coordinate schedules and patient needs through our Administrative office.
- Developing partnerships with facilities

Qualifications
- Medical degree with a focus on Primary Care Medicine or related field
- Strong communication skills for effective Patient, Family and Facility interaction
- Experience with care plans and patient management preferred
- Proficiency in EHR systems
- BC/BE
