Job description:
Standards Home Care is current servicing the following areas;
-North Shore
-South Shore
-Merrimack Valley
-Boston and its Surrounding areas
RNs/LPNs
- Provide skilled nursing care to patients in home setting.
- Assess home health patients to identify the physical, phycho-social, and environmental needs.
- Implement/develop/document the plan of care to ensure quality and continuity of care for patients.
- Provide health education to patients and/or caregivers.
- Administer prescribed medications, treatments and therapies per plan of care.
Case Manager
- Makes initial home visits, plan of care, re-certifications, and HHA care plans.
- Coordinates services, attends regular case conferences.
- Informs physician and other personnel of changes in patients condition and need.
- Counsels the patient and family for nursing and related needs.
- Participates in the in-service programs and supervises and teaches other nursing personnel and HHA's.
All nursing positions must possess:
- Strong assessment skills
- Strong organizational and problem-solving skills.
- Strong interpersonal skills
- Basic computer skills
- Well-developed written and verbal communication skills.
Experience:
- Valid Registered Nurse (RN)/Licensed Practical Nurse (LPN) license in MA
- Experience a plus
- Oasis experience preferred for Case Managers
- Valid MA drivers' license and current vehicle registration
- Must possess own vehicle
Job Types: Full-time, Part-time, Contract

Pay: $ $50.31 per hour
Expected hours: 10 – 50 per week
