Your Role:
Conduct comprehensive initial assessments for new clients to establish tailored care plans.
Perform regular reassessments to ensure ongoing quality and effectiveness of care.
Lead field orientations for caregivers, ensuring they are well-equipped to deliver exceptional service.
Collaborate closely with our administrative team to maintain compliance and accurate documentation.
Travel to clients' homes across assigned territories in New Jersey, providing hands-on nursing care and support.
What We’re Looking For:
Active RN license in the state of New Jersey (Required).
Active CPR Certification
Previous experience in home care, hospice, or community health settings is a plus.
Willingness and ability to travel to various client locations.
Strong clinical judgment, organizational skills, and compassionate communication.
Self-motivated, reliable, and capable of working independently.
