Job Duties:
- Assist individuals with personal care and self-help skills
- Support transportation and community-access needs
- Prepare meals and provide dining assistance
- Maintain a safe, clean, and comfortable living environment
- Monitor individuals' overall health, safety, and well-being
- Encourage independence and participation in daily activities
- Provide support within intermediate care facilities, community training homes, and supervised living programs
- Document services and daily activities in an electronic record-management system
- Communicate professionally with individuals, coworkers, and support teams
Requirements:
- Compassionate, dependable, and patient approach
- Strong verbal and written communication skills
- Ability to provide personal and residential care
- Basic computer skills for electronic documentation
- Ability to follow individualized support and safety plans
- Strong attention to health, safety, and accountability
- Ability to work independently and as part of a care team
