What are the responsibilities and job description for the Licensed Social Worker position at Staffing Solutions Organization LLC?
Duties:
- Provide Maine residents with disabilities with solutions to complex healthcare, financial and placement issues through integrated provider and community involvement.
- Facilitate team meetings for adults without a discharge plan until a safe location is identified or other complex issue is resolved.
- Provide member’s team with team meeting notes and update all interventions taken to the CCU database via CCU protocols.
- Provide consultation/assistance to members and their team in accessing needed services and/or other resources to ensure the least restrictive environment as possible.
- Facilitate and partner with providers to develop a member plan of care.
- Maintain collaborative relationships with community agencies, hospitals and providers in order to educate and ensure the best outcomes for the members.

- Assess all agencies for services/waivers member may be appropriate for (BIW, DD, APS, etc.).
- Perform other duties and responsibilities as may be assigned by the Department.
MINIMUM QUALIFICATIONS:
- Six (6) years of education and/or professional experience in social services casework, social services program development/delivery, or social services related fiscal/budget administration/analysis work.
- Position requires a Licensed Social Worker (LSW) license as issued by the Maine State Board of Social Worker Licensure.
