What are the responsibilities and job description for the CASE MANAGER position at St. Vincent Institution?
PURPOSE:
In accordance with the goals, objectives, and the Mission of the Daughters of Charity of St. Vincent de Paul, the Case Manager provides case management services to residents of St. Vincent’s Family Strengthening Program (FSP). Services generally include supporting and monitoring clients’ efforts in successfully achieving the objectives of a service plan and gaining access to needed community-based resources.
FAMILY STRENGTHENING PROGRAM (FSP)
The Family Strengthening Program at St. Vincent’s Santa Barbara is a drug- and alcohol-free, transitional housing program (up to 27 months), designed to assist low-income single mothers, with one or two children, five years old or younger, or pregnant with her first or second child (first child must be five years old or younger). The program provides a safe and structured environment that supports stability and self-sufficiency through case management and access to community resources.

St. Vincent's is a Roman Catholic organization sponsored and operated by the Daughters of Charity of St. Vincent de Paul, which provides programs for children, families, and seniors that are based on community needs. St. Vincent’s is the longest continuously operating human service agency in Santa Barbara. St. Vincent’s began operations in 1858.
ACCOUNTABILITY:
This position reports directly to the Director of the Family Strengthening Program.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Demonstrate a passion and commitment for the Mission and Vincentian Core Values of St. Vincent’s and the Daughters of Charity while serving as a role model in carrying out the Mission.
- Facilitate case management services for residents in the Family Strengthening Program (FSP), supporting clients in achieving goals outlined in their service plans.
- Provide at least one case management session per week per client and ensure documentation is complete, accurate, and submitted in a timely manner.
- Assist with interviewing, screening, and placing prospective residents and welcoming new residents into assigned cottages.
- Provide orientation to FSP policies, procedures, and the St. Vincent’s campus for all new residents.
- Monitor resident progress through assessments, case meetings, and ongoing documentation while supporting short and long-term goal setting.
- Conduct monthly house meetings and support residents in maintaining a safe, healthy, and respectful community living environment.
- Connect residents with appropriate community resources and agencies while supporting development of personal support systems.
- Reinforce program education classes and encourage resident participation while supporting application of classroom learning in daily life.
- Complete Enhanced Care Management (ECM) referrals, assessments, and care plans while maintaining awareness of social determinants of health.
- Ensure timely and thorough documentation of services, case notes, and ECM requirements according to program timelines.
- Maintain professional conduct, trauma-informed care practices, and strict client confidentiality in accordance with HIPAA guidelines.
QUALIFICATIONS:
- Must possess a bachelor’s degree in related field with emphasis in human development; master’s degree preferred.
- Must have at least two years of experience in a related field and familiarity/sensitivity with social issues such as domestic violence, homelessness, substance abuse, and mental illness.
- Ability to read, write and communicate effectively in English; Bilingual a plus.
- Must possess excellent interpersonal communication skills and ability to work with all levels of clients, staff, volunteers, and community members.
- Must possess exceptional time management and detail-orientation skills.
- Ability to work independently with a strong level of organizational skills.
- Proficient with PC and various software applications (Microsoft Office and other software as applicable).
- Demonstrated leadership at professional level.
- Possess the ability to generate enthusiasm and inspire confidence and support.
- Is conscientious in follow-through on job requirements and has the ability to be a self-starter.
EMPLOYMENT REQUIREMENTS
- Must pass a criminal background fingerprint check.
- Must pass a pre-employment negative TB test.
- The COVID-19 vaccination may be a requirement depending on the position within St. Vincent’s.
