Schedule: Afternoon/evening hours. Every 6th weekend and 1 federal holiday required.
PURPOSE OF POSITION: Under general supervision, carries out the resident activity program based upon State and Federal regulations. Provides activity ideas, programs, documentation and direct supervision to residents to meet the physical, mental and psychosocial well being of each resident through individual and group programs.
This position is scheduled 48 hours bi-weekly. Shift time
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
1. Contributes program ideas to enhance the effective administration of a quality resident activity program.
2. Carries out resident individual and group activity programs. Performs duties related to the preparation, clean-up and delivery of resident activity programs.
3. Promotes self-esteem and social interaction with residents and encourages participation and
involvement of all residents in activity programs.
4. Provides leisure supplies and equipment for residents’ use on assigned neighborhoods.
5. Carries out social contact for residents on a l:1 basis as defined for specific individual resident needs.
6. Assists in transport of residents to and from activity programs.
7. Encourages and accompanies residents on community outings. Transfers and assists with dining and bathroom needs of residents while on community outings.
8. Attends resident care conferences as assigned. Cooperates, supports and communicates effectively with other departments to provide residents with multidisciplinary care.
9. Carries out the Resident Activity Service Plan of Care.
10. Documents accurately, completely and legibly in accordance with policies and procedures
to include, but not limited to: quarterly MDS, progress notes, discharge, and resident activity attendance form.
11. Supports the community orientation of the department by effectively interacting with
volunteers in a respectful manner and promoting good public relations with volunteers.
12. Provides direction and supervision of volunteers in a variety of resident activity programs.
Assists in volunteer orientation and training.
PHYSICAL REQUIREMENTS TO PERFORM THE ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
50% of workday is spent standing, 30% walking and 20% sitting. Occasionally is required to twist, stoop/bend and squat as well as climb stairs. Reaching above shoulder is done on an occasional basis. Reaching at and below the shoulder are done frequently.
Is frequently required to lift and/or carry 10 – 25 lbs. (activity supplies, equipment and charts). Occasionally will lift and/or carry 25 – 50 lbs. (activity supplies and equipment). Pushes/pulls 10 – 50 lbs. frequently (activity carts); pushes/pulls 50 – 100 lbs. frequently (residents in wheelchairs).
Is able to see objects closely (activities, documentation) as well as distant objects (resident monitoring). Occasionally is required to discriminate colors (activities). Must be able to hear normal sounds and voices with or without background noise. Occasionally is required to make an assessment based upon smell (cooking). Must be able to communicate orally and/or in writing in face to face and/or group interactions. Adapts to frequent interruptions/changing situations.
