Social Worker - Skilled Nursing Facility

We are seeking to hire a Social Worker.

• $23 to $27 per hour based on circumstances including skills, qualifications, degrees and certifications, prior job experience, training, market data, and other relevant factors. 
• Full-Time schedule.

Social Worker Qualifications: 

• Bachelor's degree in Social Work or a related field required.  
• Minimum of one year of Social Work in a healthcare setting.
• Must have Skilled Nursing Facility (SNF)/Subacute experience.
• Knowledge of Point Click Care or other Electronic Medical Records (EMRs).

Social Worker Job Summary:

• Assist with documentation for the resident admissions:  Social Service Evaluation, Discharge plan, Initial Admission Progress, Geriatric Depression Scale.
• Assist with interviewing applicants and families to determine suitability of residency.
• Interview Resident with input of family to determine psycho-social strengths/weaknesses and appropriate adjustments through planned interventions.
• Update resident files/charts, Progress Notes, Plan of Care and Assessment.
• Participate in the Interdisciplinary Care Conference Meetings.
• Assist resident and/or family members with emotional and financial issues.
• Assist in communication with families.
