The Care Facilitator collaborates with attending physicians and residents and serves as advocate to help patients and families navigate the acute episode of care. The Care Facilitator monitors patients continuing stay for resource utilization and helps prepare them for a safe and timely discharge. The Care Facilitator is proactive and uses collaborative problem solving skills to meet the needs of the patients and physicians.
