Interim Clinical Program Director, Senior Homeless Services

Status: Part-time employee, Hourly,  6-8 direct reports. Licensure required.

Reports to: Senior Director of Finance and Strategic Initiatives

Compensation: $80.00/hour

Schedule: June 10 – September 15, 2026, 24 hours a week, Schedule TBD


Background: For more than five decades, St. Mary's Center has provided quality case management services to Seniors in need in West Oakland, addressing the complex and interconnected challenges facing low-income and unhoused older adults in Oakland. Grounded in a commitment to dignity, equity, and healing, our case management services provide personalized, trauma-informed support to Seniors navigating housing insecurity, chronic health conditions, mental health challenges, food instability, and social isolation. Through intensive one-on-one relationships, our skilled case managers connect participants to vital resources, housing placements, benefits enrollment, healthcare access, behavioral health services, and community programs, while also building long-term plans for stabilization and well-being.


Recognizing that many of our participants survive on less than 15% of the Area Median Income and face systemic barriers due to race, age, ability, and past trauma, our program goes beyond basic referrals. We meet each client where they are, offering holistic and culturally responsive care rooted in trust, consistency, and follow-through. By integrating services across our housing, health, advocacy, and nutrition programs, case management serves as the backbone of St. Mary’s wraparound model,

ensuring that every individual receives the tailored support they need to move from crisis to stability.



Position Summary:  Reporting to the Director of Finance and Strategic Initiatives, the Interim Clinical Program Director is responsible for managing an interdisciplinary team of professional and paraprofessionals to provide assessment, intensive case management services and advocacy for Seniors experiencing homelessness as they age and need support to achieve their highest quality of living, independence and dignity.  The role provides clinical, client-focused leadership in the agency and helps set standards of care.



Clinical Management (30%)

  • Ensure the utilization of best and evidence-based approaches to mental health care consistent with health/mental health integration, rehabilitation and recovery, wellness, treatment adherence, and housing goals, with a focus on trauma-informed care and addressing impacts of systemic racism
  • Analyze and triage current cases, managing caseload to reflect priorities, program eligibility, and capacity. Direct opening/closing of cases and manage waitlist/pipeline for case management and transitional housing (24 units)
  • Conduct daily morning huddle with team, weekly Team Meeting, and once monthly case conference to engage clinical team in best practices and protocol development, including case presentations, triage strategies, system analysis and improvement
  • Maintain effective communications with staff and partners related to seniors’ psychosocial and psychiatric needs
  • Maintain clinical records in our database in accordance with mandated standards of St. Mary’s Center, Medi-Cal and Alameda County Health, and monitor clinical files for accuracy, timeliness and clinical content to ensure compliance with contractual requirements and best practices
  • Provide crisis management and immediate therapeutic response to clients experiencing acute symptoms associated with behavioral health issues and / or referral to appropriate supports within the organization or externally when needed.
  • Link clients to appropriate community social services, including health and mental health services, entitlement, legal services, substance use treatment services and daytime activities.
  • In collaboration with other agency leaders, represent St. Mary's Center in public and professional settings to advance clinical practice and agency reputation

 

Staff Training and Development (25%)

  • Ensure case management staff attend trainings are trained and skilled in the area of medical necessity; assessment; treatment plans; progress or case notes; use of relevant databases and referral systems when scheduled.
  • Provide ongoing guidance to paraprofessional staff on clinical intervention strategies, case management planning, and behavioral health screenings for quality service delivery, including weekly individual and group supervision with clinical staff and interns in accordance with best practices and standards
  • Ensure that program staff meets quality care and productivity in accordance with contractual obligations and requirements
  • Develop and provide oversight of ongoing team development, sharing clinical knowledge, and enhancing team skills. Work with consultants or external trainers when needed on establishing/documenting clinical practice policy, case conferencing, individual and group supervision practices, and supporting related training and development

 

Quality Assurance, Contract Compliance and Reporting (25%)

Provide input to Executive Director, Clinical Housing Director and Senior Director of Finance and Strategic Initiatives on billing, scope of work, outcomes, and program deliverables during contract negotiations.

  • Review contract language for agency compliance and alignment with best practices.
  • Monitor contract outcomes/deliverables.
  • Collaborate with Contract and Data Team to ensure appropriate data is tracked and verified. Work with Clinical Housing Director and Data Team on evaluation including both program and client characteristics, i.e., evaluation outcomes based on population and service delivery
  • Support monthly, quarterly, and annual reports in collaboration with Data Team through regular data entry and recordkeeping.
  • Take the lead on Quality Assurance and Improvement strategies and implementation when appropriate


Agency Leadership (20%)

  • Serve as part of management team, participating with executive leadership in developing strategic plans
  • Act as HIPAA compliance officer, maintaining confidentiality regarding patient health information and training clinical and administrative staff as appropriate.


Skills and Duties

  • Knowledge of age-specific theories of human development; social work theory and practice as well as applicable community resources and legal aspects of care for homeless seniors
  • Knowledge of psychosocial aspects of illness including family systems theory, crisis theory and effective treatment modalities used to assist seniors in adjusting to the debilitating effects of illness
  • Adhere to professional ethics, practice and values as delineated by NASW
  • Provide culturally competent individual and group psychotherapy to seniors
  • Demonstrate knowledge, ability and willingness to engage in continuous learning about diverse ethnic, cultural, spiritual backgrounds, sexual orientation and gender identity
  • Demonstrate expertise and knowledge of DSM V and ICD-10; reporting laws and legal aspects of care; knowledge of Alameda County’s managed care system
  • Work within and promote an environment dedicated to harm reduction, recovery, and wellness.
  • For Medi-Cal eligible clients, this position performs Medi-Cal documentation and billing including assessment, diagnosis, referral, case plan development, consultation, brokerage, counseling, case management, crisis management.


Qualifications:  

  • LCSW with at least two years and qualified to provide ASW supervision hours toward licensure
  • Minimum of 2 years’ experience working with multiple co-occurring challenges of the elder population including comprehensive case management, mental health and substance use disorder counseling
  • Current, valid CA driver’s license. Own vehicle required
  • LifeScan fingerprint clearance
