What You Can Expect:
The Registered Nurse (RN) serves as a clinical resource to support patient outreach activities by providing triage services, clinical assessment, and escalation support for Outreach Coordinators. This role evaluates patient needs, prioritizes clinical concerns, and facilitates timely intervention to ensure safe, effective, and coordinated care. The RN collaborates with providers, care teams, and community resources to address patient needs, improve outcomes, and support population health initiatives.
Schedule: 1.0 Full Time (80 hours per pay period) Monday - Friday 8am to 5pm- onsite at the St. Luke's Boise Plaza 4.
Key Responsibilities
- Conduct telephonic and electronic triage of care management referrals.
- Serve as the escalation pathway for Outreach Coordinators when clinical judgment, assessment, or intervention is required.
- Assess patient risk factors and determine appropriate levels of care and follow-up.
- Coordinate with providers and interdisciplinary teams to address urgent and complex patient needs.
- Provide patient education and guidance related to symptoms, treatment plans, preventive care, and self-management.
- Document assessments, interventions, and care coordination activities in the electronic medical record.
- Support quality, care management, and population health initiatives through proactive patient engagement and clinical oversight.
- Promote patient safety, and adherence to organizational policies and clinical standards
Qualifications:
- Bachelor's of Nursing (BSN)
- Minimum 2-3 years of clinical nursing experience, ambulatory care, care management, triage, or population health highly preferred for this role.
- Strong clinical assessment, critical thinking, and communication skills.
- Ability to work collaboratively with multidisciplinary teams and manage competing priorities in a fast-paced environment
- Active Registered Nurse (RN) license
- BLS
