What You Can Expect:
- Support both Home Health and Hospice
- Completes psychosocial assessments and plans of care for home health patients.
- Identify and provide interventions and goals as indicated by the plan of care.
- Consults with physicians, healthcare professionals, the home health care team, and community partners.
- Part of the multi-disciplinary team.
- Provides community resource information to home health patients/families.
- Compensation: Paid hourly which includes drive time and mileage reimbursement for traveling or a fleet car
- Bonuses: Sign On bonus eligible
- Professional Development: Clinical Supervision and in house educational opportunities
- Schedule : Monday-Friday schedule
Qualifications:
- Education: Masters degree
- Licenses/Certifications: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in states of practice
- Other Requirements : Valid driver’s license and proof of auto insurance
