Position Summary:
Responsible for the provision of social work services to clients of the Albert Pujols Wellness Center for Adults with Down Syndrome. Assess, plan and facilitate for options and services to meet an individual's unique and complex need. This is accomplished by addressing both the individual's bio-psychosocial needs as well as the resources available. These services are offered/ provided across the continuum. Responsible for planning and implementing the INSPIRE programs which are offered to both clients and parents. Continuing commitment to maximizing the quality of life of each patient and enhancing community health.
Education, Experience, & Licensing Requirements:
Education: MSW in social work from a school accredited by the Council on Social Work Education
License: Licensed Clinical Social Worker
