St. Luke Lutheran Community is searching for an experienced Social Worker for its North Canton Campus.
Social Worker (LSW)responsibilities:
Coordinating social services with other departments through effective inter-department communication
Interviewing Residents/families to obtain history and encourage involvement in planning services as well as developing goals and objectives
Provide counseling and information/referral services to effectively meet Resident and/or family psychosocial and financial needs
Interpret social, psychological, and emotional needs of the resident/family to the medical staff, attending physical, and other resident care team members
Coordinate room changes and assist with adjustments as needed
Coordinate discharge planning
Ensure department standard operating procedures
Social Worker (LSW) Qualifications:
Social Worker's License (LSW) a plus
Recent experience in long-term care and/or geriatric social work
Must be willing to follow instructions, accept feedback, and possess the ability to make independent decisions when warranted
Must be able to communicate professionally and work effectively with personnel, Residents, family members, visitors, government agencies/personnel and other individuals from the public
Must be knowledgeable of nursing and medical practices, laws, regulations, and guidelines that pertain to long-term care
Must possess a natural leadership ability the eagerness to develop strong professional relationships on campus and within the community
