SCOPE

The Certified Nursing Assistant works under the guidance of the Director of Nursing and supervision of the licensed nurse to provide direct resident care. The Certified Nursing Assistant will serve as a team member to promote optimal resident care. In this vital role, you will contribute to our mission by providing exceptional care and support to our residents, ensuring their comfort and well-being. You are essential for providing person-centered resident care and providing a ministry of gentle service

**Per Minnesota Department of Human Services guidelines, must have successful passing of background screening. **

ACCOUNTABILITIES

Resident Direct Cares

  • Complete duties identified in each resident care plan. Duties included, but not limited to:

ü Bathing residents (bed bath, partial baths, AM/PM cares), including grooming activities (shampooing hair, oral care, nail care, skin care, shaving facial hair, etc.)

ü Feeding residents (food & fluids)

ü Toileting residents per directions in care plan

ü Dressing/undressing (including footwear)

ü Performing perineal care

ü Measuring intake & output

ü Transferring residents while utilizing appropriate supportive devices (transfer belt) and assist with ambulation using transfer belt unless resident is independent with full weight bearing ability

ü Turning & positioning residents using precautions identified by the Rehabilitation Department.

ü Transfer residents using Stand Up & Full mechanical lifts. Note: Must be 18 years or older to run the controls. If under 18, this role may be the 2nd person assisting other staff.

ü Assist Certified Nursing Assistants with resident transfers using both full & stand-up mechanical lifts.

ü Making occupied & unoccupied beds

Resident Health/Safety

  • Answer any resident's call light promptly.
  • Plan/assist with transportation of any resident to activities and therapies.
  • Maintain residents' rooms/equipment (clean, orderly, safe) & ensure adequate supplies in residents' rooms needed to complete cares.
  • Follow facility emergency plans during drills and in the event of an emergency.
  • Follow Nursing Department policy for using transfer belt.
  • Report equipment repair/maintenance needs/safety hazards.
  • Report all resident incidents and accidents immediately.
  • Report to station charge nurse any physical/behavioral changes in residents, unusual occurrences/observations, residents' response to care, and/or inability to complete assignments

Other

  • Demonstrate positive working relationships with coworkers and other departments.
  • Ensure compliance within the nursing department, organizational policies, systems, regulations and demonstrate a strong commitment to St. Luke's Lutheran Care Center's mission of providing a ministry of gentle service and person-centered care.
  • As part of the overall team, you may be requested to assist in the support of other resident care activities
  • Use effective communication with residents, families, visitors, coworkers and other health care professionals.
  • Promote positive image of St. Luke's Lutheran Care Center by embracing person-centered care with a positive attitude.
  • Wear name tag during scheduled shift

  • Adhere to the Code of Conduct and Compliance, Vulnerable Adult Policy, and any other policies in addition to other job specific policies

  • Attend scheduled mandatory in-services

PHYSICAL & SAFETY REQUIREMENTS

While performing the duties of this job, you are regularly required to walk, handle & feel objects, reach with hands & arms, reach above arm level, communicate effectively & efficiently in person & on the phone. In addition, the physical requirements for this role include, but are not limited to:

  • Ability to lift up to 60 pounds
  • Ability to carry up to 35 pounds
  • Ability to frequently bend, squat, crouch, kneel, and push/pull up to 20 pounds
  • Vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus
  • Hearing abilities required include ability to hear alarms & resident verbal direction of needs.
  • Ability to perform warm up stretches prior to shift
  • Follow standard precautions & infection control practices
  • Report any work or non-work related injury immediately
