The salary range reflected above is the normal hiring range for 2026. The full salary range, including longevity pay, is $31.21 - $44.23 per hour.
These positions are part of the Civil Service Basic Unit Employees Collective Bargaining Agreement represented by Council 5 of AFSCME.

Distinguishing Features of Work
An employee in this class is responsible for reviewing client needs as required according to federal and state program eligibility criteria. Duties may include providing comprehensive assessments, educational counseling, and providing case management and care coordination depending on the program area. The work is performed under the direction of assigned supervisory staff.

- Completing comprehensive health risk assessments at least annually for health and social needs.
- Developing, reviewing, and maintaining individualized, comprehensive care plan for clients.
- Serving as the client's advocate.

Selection Process

- Graduation from an accredited college or university with an Associate’s degree in nursing.
- Possession of a current Minnesota Registered Nurse license.
- Possession of a valid driver’s license.
HOME AND COMMUNITY – BASED & PUBLIC HEALTH MANAGED CARE PROGRAMS:
- Two (2) years full-time paid verifiable experience in a home or community-based setting.
- Possession of MnCHOICES Certified Assessors certification within two (2) months of employment. Failure to obtain the MnCHOICES Certified Assessor certification within two (2) months will result in termination from the position.
Experience & Training Rating - 100% of the scoring process:
- Points will be awarded for job-related experience and education exceeding the minimum qualifications. Only experience occurring during the ten most recent years will be used in determining this assessment. Experience beyond ten years may be included on the application for purposes of meeting minimum qualifications or informing the hiring authority of your complete employment history.
