Work location & format: 417 South Avenue, Rochester, NY 14620. Offsite work as directed at health fairs, school sites, and mobile medical unit.

Details of Key Job Responsibility Areas
1. Patient Care
- Maintains, manages, and provides primary care for an established panel of assigned patients, as well as accepts new patients
- Provides preventative health services within the confines of the center’s capability
- Perform work/school physicals
- Perform complete history and physicals on all new patients and annually as needed
- Provides supportive care indirectly to individuals via phone calls. This includes interpreting and reporting their lap/radiology results
- As NYS has Child Health Plus insurance, SJNC provides limited care for individuals under the age of 18. However, when deemed appropriate by the Director of Clinical Operations, the NP/FNP would be the designated provider for this group including but not limited to the purpose of school and work physicals
2. Record management and Referrals
- Maintains timely and complete charting and notes in the Electronic Health Record (EHR) on all assigned patients in accordance with organizational policies and procedures
- Refers patients to internal and external resources as needed to optimize patient care
3. Miscellaneous duties
- Will mentor/precept NP students primarily by precepting at least one student a year (after completion of 1 year of service)
- Employees are accountable to meet the performance standards of their departments and may be called upon to participate in required compliance audits, process improvement, and quality improvement plans

Requirements and Qualifications
- Nurse Practitioner degree (MSN) from an accredited recognized Program with license to practice in NYS and is Board Certified
- At least two years of clinical practice as an NP
- Malpractice Insurance
- Requires valid NYS driver’s license as this position may drive the organizations Mobile Outreach Unit (MOU) and other agency owned vehicles
- Requires valid NYS driver license, own reliable vehicle, and liability insurance of $100,000/$300,000 as employee will need to drive for work purposes

Physical Requirements
In compliance with New York State Department of Health (NYSDOH) infection control requirements for healthcare organizations, St. Joseph’s Neighborhood Center (SJNC) requires that all new and existing employees and volunteers provide documentation to demonstrate:
- Measles, Mumps, and Rubella (MMR) vaccination or titer
- Completion of a Tuberculosis (TB) test within six months
- Hepatitis B vaccination or declination form
- Flu vaccination or mask required in patient/client areas during flu season
- COVID 19 vaccination series or mask required in patient/client areas during prevalent respiratory virus season
- While performing the duties of this job, the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms; talk and hear
- Occasionally, the employee must stoop, bend, lift, or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus
